To get started, dog owners should fill out Section I – Contact Information on the CPT Client Contact Form, then click the DOG tab to enter Dog Info and pertinent Dog Issues sections. Once all required sections are completed, click “Send.” CPT will contact clients within 24 hours by email and/or phone.
